What is your team name?
Please provide the official name of your cheer or dance team.
Who is your head coach or team contact?
Include the full name and contact information for the person responsible for coordinating with U-FIT365.
What school or organization is your team affiliated with?
Let us know the name of your school, district, or community program.
How many athletes are on your team?
Please give an accurate estimate or confirmed number of participants.
Do you have any male athletes on your team?
This helps us tailor training, stunting, and choreography as needed.
What type of facilities do you have access to?
Indoor gym, outdoor field, mirrors, mats, sound system, etc. Please specify.
Do you have authorization from your Athletic Director to hold a camp?
Yes or No — formal approval may be required before scheduling.
Are you interested in a Private Camp or Team Camp?
